Our application process was thoughtfully created to obtain the documentation needed for our financial grants while seeking to minimize additional stress and hardship for those applying. We understand that some of the information requested may be difficult to revisit during such a painful time. As you move through this process, we encourage you to give yourself the space and grace needed to care for yourself and your loved ones.

Please note, we require identify verification and criminal background check for all applicants. Additional documents required at the time of application are as follows, a copy of the decedent’s death certificate and the police report. If you do not yet have a death certificate issued yet, you will need to provide a copy of the funeral home invoice or we will contact your service provider once consent is given.
